I am a patient at Eastmoor Health Centre, will I have to re-register?
No, there is no need to re-register and you can continue to use the Practice as usual
Can I register as a new patient?
Yes, we currently have an open list and are accepting new patient
What will the telephone number be?
The telephone number will remain the same, which is 01924 201614
When will the surgery be open?
There are no changes to the Practices opening times which are Monday to Friday 8am to 6.30pm
What happens if I need a GP outside normal hours?
In Wakefield, you can get face-to-face appointments for urgent problems and book routine appointments in advance through the GP Care Wakefield service. The service is available between 5pm and 9.30pm on weekdays; 9am and 5pm on Saturdays; and 9am and 1pm on Sundays and 9am and 3pm on bank holidays. All you need to do is contact the Practice on the usual telephone number, at any time of day and you will get through to the right service
How do I make an appointment?
You can make appointments with the Practice via phone or in person. We also invite patients via SMS to make an appointment online for specific reasons such as smears, blood tests, vaccinations etc
What do I do if I need a home visit?
Contact the surgery as normal and the Reception will send your request to a GP
How do I order my repeat prescriptions?
Via the NHS App or your patient online account (SystmOnline). You can also order in person at reception. We do not take 3rd party requests from pharmacies on your behalf
How do I collect my prescriptions?
If you nominate a local pharmacy, we will electronically transfer the prescription to that pharmacy. If your pharmacy normally delivers to your home, they will continue to do so. Otherwise, please collect your prescription in the normal way
How do I get my test results?
Test results are available everyday from 11am by contacting the Practice in person or by phone. This is to allow staff to deal with same day appointments between 8am and 11am
What will happen about my annual reviews?
Patients will now be invited for their annual review in the month that they were born for e.g., if you are born in August, you will be invited in August. The Practice will contact you when its your turn to be seen
What will happen about baby and childhood immunisations?
We will contact eligible patients and invite them in to see the Practice Nurse
What will happen about my annual flu vaccination?
We will start to arrange flu clinics from September onwards. Patients who are eligible will be sent an SMS with an invite to make an appointment online. Patients unable to do this will be contacted by telephone or letter
Will I be able to join a Patient Participation Group (PPG)?
The Practice intends to set up a PPG to meet with patients to discuss practice issues and patient experience to help improve the service. Ask at reception if wish to be involved
How will you keep in touch with patients?
Facebook (@eastmoorhealth) or our website (www.eastmoorhc.nhs.uk). We may send SMS occasionally, but please let us know if you would like to opt out of these. We may also contact you by telephone as usual
Will any staff be made redundant?
No, all staff employment is protected and transferred into the new partnership
