New Patients

We can only accept new patients who live in our practice catchment area – marked in black on the Practice Boundary Map. If moving out of this ‘inner’ boundary you can still remain registered if you have relocated to within the outer boundary (marked in red), otherwise, you will be asked to register with another practice.

You can register by completing a registration form which can be obtained from the practice reception. When you register, we will ask you to make an appointment to see the Health Care Assistant for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

Practice Boundary

The Practice will accept patients from the Eastmoor Estate and from the surrounding area from Newton Hill, St. John’s, Leeds Road – as far as the boundary with Outwood, Stanley – as far as the War Memorial on Aberford Road.  If you are unsure if your house falls into our catchment area, please check with Reception.
